Instructions on Submitting and Paying for Your Advertisement

  1.  Download, Complete, Sign the agreement.

  2. Fax the signed and completed agreement to 646-219-0009 or scan and e-mail it to info@zascianek.org

  3.  Send your art work and any other comments to info@zascianek.org

  4.  Make Payment for the ad based on the ad size price schedule listed in Paragraph 12 of the agreement.
